Choosing a new health plan for 2027? Make sure the plan includes the primary care provider you want to see. Provena Care accepts many ACA Marketplace and Medicare Advantage plans, and we can check a plan for you before you enroll.
How to keep your primary care provider when you pick a plan
Many Marketplace and Medicare Advantage plans in Miami-Dade are HMOs that ask you to choose a primary care provider. A few minutes of checking before you enroll can save you from switching doctors later.
- Check the plan's provider directorySearch for Provena Care or Aquiles Garcia-Menocal, APRN, in the directory for the exact plan you are considering, not just the carrier.
- Call us with the plan nameCall (305) 395-7108 with the plan name and ID if you have one. We will tell you whether we participate in that plan.
- Name your primary care providerIf the plan asks you to pick a primary care provider, choose the Provena Care provider listed in your plan's directory, at 10251 SW 72nd St, Suite 106, Miami, FL 33173.
- Book your first visit earlyOnce your coverage starts, book an annual wellness visit so we have your history, medications and labs on file.

- Choose us as your providerIf the plan asks for a primary care provider, choose the Provena Care provider listed in your plan's directory.
- Book your first visitCall (305) 395-7108. We verify your coverage and find a time, often the same week.
- Ongoing careAnnual wellness visit, chronic care, labs and follow-ups, in the office or by telehealth in Florida.

We accept many plans of both types. Coverage and costs depend on the plan you choose.
ACA Marketplace plans
Individual and family plans bought through HealthCare.gov. We accept many Marketplace plans. Coverage varies by plan, so call to confirm yours.
Medicare Advantage plans
We accept many Medicare Advantage plans. Coverage varies by plan, so call (305) 395-7108 with your plan name to confirm before you enroll or switch.
Questions patients ask
When is open enrollment for 2027?
Medicare open enrollment runs October 15 to December 7. ACA Marketplace open enrollment starts November 1, and you need to enroll by December 15 for coverage that starts January 1. Check HealthCare.gov for the final Marketplace deadline.
Does Provena Care accept Marketplace plans?
Yes. We accept many ACA Marketplace plans. Not every plan is included, so call (305) 395-7108 to confirm the exact plan before you enroll.
Do you accept Obamacare plans?
Yes. Obamacare is another name for ACA Marketplace coverage, and we accept many Marketplace plans. Not every plan is included, so call (305) 395-7108 to confirm the exact plan before you enroll.
Does Provena Care accept Medicare Advantage?
Yes. We accept many Medicare Advantage plans. Call us with your plan name to confirm before you enroll or switch.
Who would be my primary care provider?
Aquiles Garcia-Menocal, APRN, is our primary care provider, with Dr. Victor Calvo, MD, serving as medical director. Both see patients in English and Spanish.
Can you help me choose a plan?

Do I need a membership if I have insurance?
No. Membership is optional. Insured patients can use their plan for primary care without joining.
