Is there a contract or commitment?
All memberships require a $199 initiation fee, are month-to-month, and can be cancelled at any time with no penalty. For most tiers, a contract may replace initiation fee if requested.
Can I upgrade or downgrade my tier?
Yes. You can change your membership tier at any time. Changes take effect at the start of your next billing cycle.
Does my membership replace insurance?
Provena operates on a direct-pay membership model. We recommend keeping your insurance for hospitalizations and specialist referrals outside our scope.
What if I need labs not included in my panel?
Additional labs are available at member-preferred pricing through our lab partner network — significantly below standard retail rates.
Is telehealth available same day?
Core members receive same/next-business-day access. Performance and Elite members receive priority same-day scheduling.
